About the Role

We are seeking a highly skilled and compassionate Social Worker to join our Neighborhood PACE program. As a key member of our Interdisciplinary Team, you will play a vital role in developing plans of care for participants who want to live safely in their communities.

Responsibilities
  • Conduct comprehensive psychosocial assessments to identify factors impacting participants' current level of functioning.
  • Formulate treatment plans in collaboration with the Interdisciplinary Team, recognizing the ways PACE can maintain or enhance participants' well-being and level of functioning in the community.
  • Provide caregiver support, coordinate family meetings, and demonstrate leadership in these efforts.
  • Participate actively as part of the Interdisciplinary Team, sharing assessments and contributing to the care plan, collaborating with participants to identify their goals of care.
  • Develop and lead activities demonstrating clinical expertise, such as group counseling, resource management, psychotherapy, end-of-life care, and supervision of social work interns.
  • Coordinate admissions and discharges, and provide care coordination for participants in need of psychiatric hospitalization.
Requirements

We are looking for a candidate with an MSW and a passion for serving adults 55+. Experience working with older adults is preferred. Must possess or be working toward licensure as a Licensed Certified Social Worker (LCSW) or Licensed Independent Clinical Social Worker (LICSW) in MA. Bilingual (Spanish speaking) is required.
