About the Role

We are seeking a highly skilled PACE Social Worker to join our dynamic Interdisciplinary Team (IDT) at NeighborHealth, formerly East Boston Neighborhood Health Center. As a PACE Social Worker, you will play a vital role in developing plans of care for participants who want to live safely in their communities.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify factors impacting participants' current level of functioning.
  • Formulate treatment plans in collaboration with the IDT, recognizing the ways PACE can maintain or enhance participants' well-being and level of functioning in the community.
  • Provide caregiver support, coordinate family meetings, and demonstrate leadership in care coordination.
  • Participate actively as part of the IDT, sharing assessments and contributing to the care plan, collaborating with participants to identify their goals of care.
  • Develop and lead activities demonstrating clinical expertise, such as group counseling, resource management, psychotherapy, or case presentation at the ethics committee.
  • Coordinate admissions and discharges, and provide care coordination for participants in need of psychiatric hospitalization.
Requirements

We are seeking a candidate with an MSW and licensure as a Licensed Certified Social Worker (LCSW) or Licensed Independent Clinical Social Worker (LICSW) in MA. Bilingual (Spanish speaking) is required. Experience working with older adults is preferred.
