About the Role

We are seeking a highly skilled and compassionate PACE Social Worker to join our team at East Boston Neighborhood Health Center. As a PACE Social Worker, you will play a vital role in providing exceptional patient care and support to our participants.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify factors impacting participants' current level of functioning.
  • Develop and implement individualized plans of care to maintain or enhance participants' well-being and level of functioning in the community.
  • Provide caregiver support and coordinate family meetings to ensure seamless care coordination.
  • Participate actively as part of an Interdisciplinary team, sharing assessments and contributing to the development of treatment interventions.
  • Develop and lead activities that demonstrate clinical expertise, such as group counseling, resource management, and psychotherapy.
  • Coordinate admissions and discharges, and provide care coordination for participants in need of psychiatric hospitalization.
Requirements
  • Master's degree in Social Work (MSW) from an accredited institution.
  • Passion for serving adults 55+ and experience working with older adults preferred.
  • Licensure as a Licensed Certified Social Worker (LCSW) or Licensed Independent Clinical Social Worker (LICSW) in MA required.
  • Bilingual (Spanish speaking) required.
