About the Role

We are seeking a highly skilled and compassionate PACE Social Worker to join our dynamic Interdisciplinary Team (IDT) in our innovative model of care. As a PACE Social Worker, you will play a vital role in developing plans of care for participants who want to live safely in their communities.

Key Responsibilities
  • Conduct psychosocial assessments to identify factors impacting participants' current level of functioning.
  • Formulate treatment plans and provide counseling to support participants achieving their goals.
  • Work collaboratively with the IDT to coordinate care and address issues such as high-risk mental health, end-of-life care, medical decision-making, and caregiver support.
  • Develop and lead activities that demonstrate clinical expertise, such as group counseling, resource management, and psychotherapy.
  • Coordinate admissions and discharges, and provide care coordination for participants in need of psychiatric hospitalization.
Requirements
  • Master's degree in Social Work (MSW) and licensure as a Licensed Certified Social Worker (LCSW) or Licensed Independent Clinical Social Worker (LICSW) in MA.
  • Experience working with older adults preferred.
  • Bilingual (Spanish speaking) required.
