We are seeking an experienced clinical pharmacist to join our integrated heart failure team at Guy's and St Thomas' NHS Foundation Trust. As a key member of the team, you will play a vital role in delivering pharmacist-led heart failure medicines optimisation clinics, utilising independent prescribing skills. You will also attend in-patient heart failure consultant-led ward rounds, collaborating with the multi-disciplinary team to ensure safe and effective medicines use.
Key Responsibilities- Develop and deliver clinical pharmacy services for patients with cardiovascular disease, with a specific focus on heart failure.
- Lead the development of pharmacist-led heart failure medicines optimisation clinics, ensuring safe and effective use of medications.
- Collaborate with the multi-disciplinary team to ensure seamless care for patients with heart failure.
- Contribute to the development of clinical governance agendas related to drug use, ensuring safe and effective use of medications.
The Cardiovascular service at Guy's and St Thomas' is one of the largest in the UK, providing a wide range of services to the local population and specialist services to patients across South London and Kent. This is an exciting opportunity to join a forward-thinking team and contribute to the development of clinical pharmacy services in heart failure.
Person Specification- MPharm degree
- Registration with GPhC
- Higher degree / Postgraduate Diploma in General Pharmacy Practice, or equivalent
- Independent Prescribing Qualification
