The Homerton University Hospital Heart Failure Service is a comprehensive care program designed to provide evidence-based treatment for patients with heart failure. As a Clinical Nurse Specialist in Cardiology, you will play a crucial role in leading the implementation of National Guidelines and Quality Standards for chronic heart failure care.
Main DutiesAs a key member of the ACERS team, you will be responsible for:
- Supporting heart failure patients in the community to optimize their treatment and manage their condition
- Preventing unplanned admissions and ensuring a seamless pathway for patients through different aspects of the service
- Working closely with the hospital team and other agencies to provide comprehensive care
- Developing and implementing personal management plans for patients and educating them on self-care
To be successful in this role, you will need:
- A strong background in cardiology with relevant training and experience
- Excellent communication and clinical assessment skills
- Experience of caring for patients with chronic or acute heart failure
- Significant post-registration experience in cardiology
- Understanding and demonstration of the Trust Values
