Job Overview

The Homerton University Hospital Heart Failure Service is a tailored approach to providing evidence-based care for patients with heart failure. As a Clinical Nurse Specialist, you will be responsible for leading specific areas of the service to ensure a seamless pathway for patients. This includes optimizing heart failure treatment, supporting patients to manage their condition, and preventing unplanned admissions.

Main Duties of the Job

We are seeking an experienced and enthusiastic nurse to join our team as a key link in the chain of gold standard care. Essential criteria include a strong background in cardiology, relevant training, and excellent communication skills. You will develop personal management plans, educate and empower patients and carers, and work closely with the hospital team.

Person Specification

  • RGN/RN
  • Post-registration nursing course in cardiac or heart failure care
  • Recognized teaching certificate
  • Clinical assessment skills
  • Experience of caring for patients with chronic or acute heart failure
  • Venepuncture trained and proficient
  • Significant post-registration experience in cardiology
  • Understanding and demonstration of the Trust Values
