Highly Specialist Pharmacist: Heart Failure Specialist

This is an exciting opportunity for an experienced clinical pharmacist to develop within the specialty of heart failure working within the integrated heart failure team at Guy's and St Thomas' NHS Foundation Trust.
Main Duties- Act as a clinical role model for pharmacists involved in the care of patients with cardiovascular disease (with a specific focus on heart failure) and demonstrate the ability to provide safe, clinically effective and cost-efficient use of drugs in these patients.
- Support clinical pharmacy services for patients with cardiovascular disease (with a specific focus on heart failure) in accordance with priorities set by the post holder, lead heart failure consultant and lead cardiology pharmacist, in order to ensure safe, clinically effective and cost-efficient use of drugs in these patients.
- Monitor clinical pharmacy services for patients with cardiovascular disease (with a specific focus on heart failure) in accordance with the priorities set by the post holder, lead heart failure consultant and general manager/clinical director in order to ensure safe, clinically effective and cost-efficient use of drugs in these patients.
The Cardiovascular service at Guy's and St Thomas' is one of the largest in the UK and consists of three main departments: Cardiology; Cardiac Surgery; and Vascular Surgery. All three provide a wide range of services to the local population as well as providing specialist services to patients across South London and Kent.
Job Responsibilities- The safe and clinically effective use of medicines in patients with cardiovascular disease (with a specific focus on heart failure)

- To work collaboratively with senior pharmacy and medical staff, the multidisciplinary teams and management and to participate in the national agenda for pharmacy services in accordance with the agreed objectives
