Highly Specialist Pharmacist: Heart Failure Specialist

We are seeking an experienced clinical pharmacist to join our integrated heart failure team at Guy's and St Thomas' NHS Foundation Trust. As a Highly Specialist Pharmacist: Heart Failure, you will play a key role in delivering pharmacist-led heart failure medicines optimisation clinics, working collaboratively with the multi-disciplinary team to ensure safe and effective medicines use.
Key Responsibilities- Act as a clinical role model for pharmacists involved in the care of patients with cardiovascular disease, with a specific focus on heart failure.
- Deliver pharmaceutical services to acute and/or specialist medicine patients, with a focus on heart failure.
- Support clinical pharmacy services for patients with cardiovascular disease, with a specific focus on heart failure, in accordance with priorities set by the post holder, lead heart failure consultant, and lead cardiology pharmacist.
- Monitor clinical pharmacy services for patients with cardiovascular disease, with a specific focus on heart failure, in accordance with the priorities set by the post holder, lead heart failure consultant, and general manager/clinical director.
The Cardiovascular service at Guy's and St Thomas' is one of the largest in the UK, consisting of three main departments: Cardiology; Cardiac Surgery; and Vascular Surgery. We provide a wide range of services to the local population, as well as specialist services to patients across South London and Kent.
Person Specification- MPharm degree
- Registration with GPhC
- Higher degree/Postgraduate Diploma in General Pharmacy Practice, or equivalent
- Independent Prescribing Qualification
We are looking for a motivated and experienced clinical pharmacist who can demonstrate a critical approach to the development and evaluation of clinical pharmacy services. The successful candidate will have a recognised postgraduate qualification with a research component, or equivalent published evidence, and be able to integrate research evidence into practice.
The ideal candidate will have a strong track record of delivering clinical pharmacy services in cardiovascular medicine, with a specific focus on heart failure, and be able to motivate and inspire others to improve the ward and clinical services provided.
