The Role:

As a qualified Occupational Therapist, you will lead holistic assessments, plan and review interventions to support individuals in maintaining or regaining independence in daily activities. You will be part of a multi-disciplinary Reablement Service team, working collaboratively to deliver a strengths-based, short-term service.

Key Responsibilities:

  • Conducting person-centred functional analysis and interventions to support service users.
  • Collaborating with the Reablement Service team, including Occupational Therapists, practitioners, carers and business support.
  • Providing expertise and support to staff, informed by social care legislation and clinical knowledge.
  • Working independently and as part of a team, with a focus on delivering high-quality outcomes for service users.

Requirements:

  • Occupational Therapy qualification and professional registration with HCPC.
  • Experience in assessment, planning and reviewing of case work.
  • Ability to work independently and collaboratively, escalating cases as required.
  • Commitment to demonstrating the values and behaviours of the Shared Services Partnership.
