About the Role:
We are seeking a qualified Occupational Therapist to join our Reablement Service team in Hampshire County Council. As an Occupational Therapist, you will play a vital role in helping individuals maintain or regain independence in daily activities.
Key Responsibilities:
- Conduct holistic assessments and develop person-centred plans to support service users.
- Collaborate with a diverse team, including Occupational Therapists, practitioners, carers, and sensory staff, to provide comprehensive support.
- Work independently, providing expertise and support to staff, informed by social care legislation and clinical knowledge.
- Lead on the assessment, planning, and reviewing of complex case work, ensuring effective interventions and outcomes.
- Conduct person-centred functional analysis and interventions to help service users maintain or regain independence in daily activities.
Requirements:
- Occupational Therapy qualification and professional registration with HCPC.
- Experience in assessment, planning, and reviewing of case work, with a focus on complex cases.
- Ability to work independently and collaboratively, escalating cases as required.
- Commitment to demonstrating our values and behaviours, including a focus on person-centred care.
- Understanding of the statutory duties of local government in social care and the social care model.
- Some experience of supervising staff, with a focus on supporting and developing team members.
