The Role:

We are seeking a skilled Occupational Therapist to join our Reablement Service team. As a key member of our team, you will conduct home visits to lead holistic assessments, plan and review interventions, and provide support across our multi-disciplinary service. You will work collaboratively with a diverse team, including Occupational Therapists, practitioners, carers, sensory staff, and business support.

Key Responsibilities:
  • Leading on the assessment, planning, and reviewing of complex case work.
  • Conducting person-centred functional analysis and interventions to help service users maintain or regain independence in daily activities.
  • Collaborating with a diverse team to provide holistic support to service users.
  • Working independently, providing expertise and support to staff, informed by social care legislation and clinical knowledge.
  • Rotational weekend work may be required.
Requirements:
  • Occupational Therapy qualification and professional registration with HCPC.
  • Experience in assessment, planning, and reviewing of case work.
  • Ability to work independently and collaboratively, escalating cases as required.
  • Commitment to demonstrating our values and behaviours.
  • Understanding of the statutory duties of local government in social care and the social care model.
  • Some experience of supervising staff.
