Job Title: Domiciliary Care Team Leader

We are seeking an experienced and compassionate Domiciliary Care Team Leader to join our team at St Georges Home Care Ltd. As a key member of our care delivery team, you will be responsible for leading our team of care workers and ensuring that our clients receive the highest level of care and support in their own homes.

The successful candidate will have a minimum of one year's experience in field care supervision or two years in health and social care. A clean UK driving licence is essential, with a strong preference for a car driver. You will also need to hold a completed QCF Level 3 in Health and Social Care or equivalent qualification and upwards.

Key Responsibilities:

  • Leading and managing a team of care workers in the field;
  • Conducting assessments and person-centred, outcome-based care plans for new service users;
  • Carrying out risk assessments as part of the individual needs assessment and update these as required as part of the service user's risk management;
  • Monitoring, supervising and mentoring all care workers in the field;
  • Liaising closely with care coordinators regarding recruitment, permanent allocation of care & support workers and the writing of care & support plans;
  • Reporting any changes in a service user to the care coordinator;
  • Ensuring the accuracy and completeness of all documentation held in service users' homes;
  • Having at least one year's experience of field care supervision OR at least two year's experience of health and social care;
  • Holding a clean UK driving licence;
  • Being a strong car driver;
  • Completing QCF Level 3 in Health and Social Care or equivalent qualification and upwards.

Salary: £32,000 - £38,000 per annum (dependent on experience)
