About the Role:

We are seeking a highly skilled Specialist Social Worker to join our High Needs Team in Kingston upon Hull. As a key member of our team, you will be responsible for delivering high-quality services to individuals with complex needs.

Main Responsibilities:
  1. Conduct comprehensive assessments to identify the needs of service users.
  2. Develop and implement person-centred care plans in collaboration with multidisciplinary teams.
  3. Maintain accurate records in accordance with legal and regulatory requirements.

Requirements:

  1. Qualified Social Worker with registration in the UK.
  2. Experience working with individuals with mental health issues and learning disabilities.
  3. Strong assessment, intervention, and care planning skills.
  4. Commitment to continuous professional development.
