About the Role

We are seeking a highly skilled and compassionate Senior Social Worker to join our Adult Assessment and Safeguarding Team. As a key member of our team, you will play a vital role in assessing the needs of vulnerable adults and ensuring their safety and well-being.

Key Responsibilities
  • Conduct comprehensive assessments under the Care Act 2014 to identify needs, risks, and eligibility for services, focusing on vulnerable adults and those at risk of harm.
  • Identify and address safeguarding concerns, ensuring timely intervention to protect adults from abuse, neglect, or exploitation.
  • Develop and implement person-centered care plans that promote independence, enhance well-being, and ensure safety.
  • Collaborate with health professionals, housing services, community organisations, and other stakeholders to provide coordinated, multi-agency responses.
  • Lead safeguarding investigations, implementing appropriate protection plans and following up to ensure ongoing safety and risk management.
  • Regularly review and update care and safeguarding plans to adapt to any changing needs or circumstances.
  • Advocate for the rights, well-being, and dignity of vulnerable adults, ensuring they have access to necessary services, resources, and support networks.
Requirements
  • 2 years experience post qualification
