Job Title: Manager - Outreach Service for Adults with Learning Disabilities

We are seeking an experienced and passionate Manager to lead our outreach service in South Camden, providing support to adults with mild-moderate learning disabilities and complex needs.

About the Role:

The successful candidate will oversee the development and implementation of care plans, coordinating and providing excellent person-centred care and support to service users. This will enable individuals to live independently in their own homes and receive support in all areas of day-to-day living.

Key Responsibilities:
  • Develop and implement care plans to provide meaningful choice and a range of activities and support to people with learning disabilities.
  • Coordinate and provide excellent person-centred care and support to service users, enabling them to live independently.
  • Oversee the development and implementation of new activity groups focusing on social interaction, learning, and general wellbeing for service users with learning disabilities.
  • Lead and coach a team supporting people with learning disabilities or another vulnerable client group.
Requirements:

The ideal candidate will have experience of leading and coaching a team, supporting people with learning disabilities or another vulnerable client group. Experience of supporting people with high and complex support needs would be an advantage.
