Job Summary:

We are seeking a highly motivated and experienced Family Support Worker to join our team at Together for Children Sunderland. As a Family Support Worker, you will play a vital role in delivering activities and support to isolated and vulnerable families, working directly with children, young people, and families in the home and across the Family Hubs Network.

Key Responsibilities:

  • Deliver activities and support to isolated and vulnerable families, working directly with children, young people, and families in the home and across the Family Hubs Network.
  • Work directly with children, young people, and families to identify those who would benefit from early help support and provide targeted interventions to improve outcomes.
  • Support families on the Enhanced Pathway for Parents in partnership with Health Visitors and midwifery, as well as being part of a Team Around a Family when appropriate.
  • Improve outcomes for vulnerable children, young people, and their families specifically in relation to the Company's Key Performance Indicators and the DfE ask in terms of Family Hubs, in particular around the home learning environment, parenting, and infant feeding.
  • Work collaboratively with other agencies and professionals to ensure a seamless service delivery and achieve the best possible outcomes for families.

Requirements:

  • Experience of working with vulnerable families and children.
  • Ability to work effectively in a team and build strong relationships with colleagues, partners, and families.
  • Excellent communication and interpersonal skills.
  • Ability to work flexibly and adapt to changing circumstances.
  • DBS clearance.
