Cumbria, Northumberland, Tyne and Wear NHS Foundation Trust is seeking a skilled and experienced professional to join our team as a Lead, Community Management.

About the Role

This is an exciting opportunity to lead a team of healthcare professionals in providing high-quality community-based services to our patients. As a Lead, Community Management, you will be responsible for monitoring the performance and quality of the service, providing clinical leadership, and managing the team effectively.

Main Responsibilities
  • Lead and coordinate team communication, ensuring effective information sharing between the team, senior management, and external agencies.
  • Support the team in planning and adjusting the service in response to new initiatives and ways of working.
  • Monitor the performance and quality of the service, identifying areas for improvement and implementing changes as needed.
  • Provide clinical leadership and guidance to the team, ensuring that they have the necessary skills and knowledge to deliver high-quality care.
Requirements
  • First Level Registered Nurse or equivalent qualification.
  • Relevant post-graduate qualification or training.
  • Experience of coordinating and managing staff resources, working within a multidisciplinary team, and providing community-based mental health services.
  • Leadership skills, with the ability to delegate and motivate the team.
  • Excellent communication and interpersonal skills, with the ability to work effectively with colleagues, patients, and external agencies.
