As a Complex Needs Care Worker, you will play a vital role in providing individualized care and support to service users, promoting independence and empowering them to achieve their goals. You will work closely with colleagues and other stakeholders to ensure seamless delivery of care and services.

Key Responsibilities:
- To provide individualized care and support to service users with complex needs.
- To work collaboratively with colleagues and other stakeholders to deliver seamless care and services.
- To maintain accurate and up-to-date records of service user interactions, including progress notes, risk assessments, and care plans.
- To communicate effectively with service users, colleagues, and other stakeholders, ensuring clear understanding and expectations.
- To contribute to a culture of quality and excellence within the organization, promoting best practices and continuous improvement.
Requirements:
- Experience working with individuals with complex needs.
- Strong understanding of care planning and risk assessment processes.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively as part of a multidisciplinary team.
- Knowledge of relevant legislation and regulations governing adult social care.
Salary: £25,272 per annum (pro rata)
