Job Title: Accident and Emergency Nurse

We are seeking an experienced and skilled Accident and Emergency Nurse to join our team at Livewell Southwest. As a key member of our multidisciplinary team, you will be responsible for providing high-quality patient care and support to individuals in crisis situations.

Key Responsibilities:
  • Assess, diagnose, and implement complex care plans for patients with a wide range of conditions.
  • Act as a Care Coordinator, arranging for appropriate care packages and collaborating with patients, carers, and multi-professional agencies.
  • Provide clinical advice and information to patients, carers, and healthcare professionals, ensuring effective communication and concordance.
  • Develop and maintain effective working relationships with multidisciplinary teams, delivering a cost-effective and high-quality service to patients and carers.
  • Contribute to health promotion and disease management initiatives, monitoring and improving community health standards.
  • Participate in policy development, implementation, and monitoring, ensuring clinical practice reflects national and local drivers.
  • Supervise, monitor, and develop standards for care delivered to patients and carers, ensuring maximum independence and quality of life.
  • Lead complex/urgent situations, providing emotional support and guidance to patients and carers.
  • Undertake comprehensive risk assessments, ensuring patient, carer, and team member safety.
  • Report and record incidents, near misses, and health, safety, security, fire, physical violence, aggression, and verbal abuse.
  • Maintain accurate records, adhering to organisational policies, professional guidelines, and Government directives.
  • Comply with infection control policies, attend relevant updates, and report issues to the immediate line manager.
  • Monitor and maintain the health, safety, and security of self and others.
  • Act as a clinical advisor on healthcare issues within their area of expertise.
  • Undertake health promotion activities, contributing to planned health promotion initiatives.
  • Develop links with other professional and voluntary bodies to ensure maximum effectiveness of the service and patient care.
  • Work alongside Community Practitioners to monitor complex health needs and reduce hospital admissions.
  • Implement, participate, and facilitate Clinical Supervision as per organisational Policy.
  • Be conversant with vulnerable adult and child protection procedures and policies of the organisation.
  • Manage a caseload team without direct supervision, participating in policy development and implementation.
  • Support, complete, and monitor staff to ensure submission of necessary forms, documentation, and IT data.
  • Have a flexible approach to the working day, meeting patient and service needs.
  • Coordinate annual leave, study leave, and duty rotas.
  • Monitor sickness, line management, appraisals, mandatory training, standards, and staff performance.
  • Investigate complaints with support from the CCRT Manager and Customer Services Department.
  • Maintain lone working information for identified staff group and monitor staff adherence to lone working policy and use of devices.
  • Maintain own clinical professional development (CPD) by keeping abreast of new trends and developments, National Service Frameworks, and guidelines.
  • Use a computer for timely recording patient activities, dealing with e-mail queries, stock and equipment ordering.
  • Preserve confidentiality and be aware of the Data Protection Act, Access to Health Records, and Consent for Treatment.
  • Use a mobile phone and lone working device for work purposes, adhering to organisational policy.
  • Assume responsibility for own professional development and personal knowledge.
