About the Role

Livewell Southwest CIC is seeking a highly skilled and compassionate Registered Mental Health Nurse to join our Community Crisis Response Team (CCRT). As a key member of our interdisciplinary team, you will play a vital role in supporting clients at home, avoiding hospital admission and promoting positive risk-taking.

Key Responsibilities
  • Assess, diagnose, implement, plan and review complex needs and packages of care in partnership with patients, carers and multi-professional agencies.
  • Provide holistic care and demonstrate positive leadership skills, working within the framework of the CCRT.
  • Undertake comprehensive risk assessments of all situations associated with the care of patients to ensure the safety of patients, carers, and team members.
  • Report and record all incidents and near misses relating to health, safety, security, fire, physical violence, aggression and verbal abuse.
  • Monitor and maintain the health, safety and security of self and others.
Requirements
  • Registered Mental Health Nurse with a can-do attitude and a passion for delivering high-quality care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and multi-professional agencies.
  • Ability to work autonomously and as part of a large multidisciplinary team.
  • Strong assessment and problem-solving skills, with the ability to think critically and make informed decisions.
  • Knowledge of relevant policies, procedures and guidelines, including the NMC Code and Data Protection Act.
