Job Summary

We are seeking an experienced and skilled Accident and Emergency Nurse to join our team at Livewell Southwest. As a key member of our multidisciplinary team, you will be responsible for providing high-quality clinical care to patients in crisis situations.

Key Responsibilities
  • Assess, diagnose, and implement complex care packages for patients with multiple needs.
  • Act as a Care Coordinator, arranging for appropriate care packages to be set up and ensuring seamless transitions between services.
  • Provide clinical leadership and guidance to the multidisciplinary team, promoting evidence-based practice and quality care.
  • Develop and maintain strong relationships with patients, carers, and other healthcare professionals to ensure effective communication and collaboration.
  • Contribute to the development and implementation of health promotion initiatives and disease management programs.
  • Participate in policy development and implementation, ensuring that clinical practice reflects national and local drivers.
  • Supervise and monitor the delivery of care by the multidisciplinary team, ensuring that standards are met and quality care is provided.
  • Take a lead role in complex and urgent situations, providing emotional support and guidance to patients and carers.
  • Undertake comprehensive risk assessments and report incidents and near misses, ensuring the safety of patients, carers, and team members.
  • Maintain accurate and confidential records, adhering to organizational policies and professional guidelines.
Requirements
  • Registered Nurse with a valid registration and a minimum of 2 years' experience in Accident and Emergency nursing.
  • Strong clinical skills and knowledge of emergency care, including assessment, diagnosis, and treatment of complex conditions.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and multidisciplinary teams.
  • Ability to provide clinical leadership and guidance, promoting evidence-based practice and quality care.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Ability to work independently and as part of a team, with a flexible approach to working hours and duties.
