Job Summary:
We are seeking a skilled Disability Coach to join our team at Cardiff and Vale University Health Board. This role will involve supporting the multidisciplinary team in delivering rehabilitation services across 7 days, including weekends and bank holidays.

Job Responsibilities:
The successful candidate will be able to: Organize delegated workload to support direct patient care; Prepare and plan own groups in line with the Rehabilitation plan; Demonstrate awareness of disability/impairments; And more.

Required Skills and Qualifications:
A good standard of education, GCSE English and Maths (Grade A-C) or Level II equivalent qualification; NVQ/QCF Level 4 or equivalent level of knowledge or NVQ/QCF Level 3 PLUS Additional training to Diploma level or equivalent; Relevant experience of working in a rehabilitation environment;

Estimated Salary: £25,549 - £31,383 per annum.
