Adult Day Centre Care Coordinator

**Care Coordinator Opportunity:**
Join us at the Adult Day Centre in Guildford as a Care Coordinator and make a real difference in the lives of autistic learners aged 18 and up. As a key member of our staff, you will play a vital role in supporting our learners, fostering their skills and knowledge, and promoting their independence and social participation.
The successful candidate will work one-on-one and in small groups with our learners, implementing behaviour-based teaching strategies and interventions to encourage positive outcomes and growth. They will also assist learners in applying their skills in practical settings, both within the Centre for Lifelong Learning and in the wider community.
Key Responsibilities:
- Provide dedicated care, support, and education to autistic adult learners.
- Plan, prepare, and deliver lessons and activities that build essential knowledge and skills.
- Offer behaviourally-based assistance to reduce behaviours that impact quality of life.
- Support learners in maintaining personal hygiene and dignity, including assistance with toileting, dressing, and washing.
- Facilitate participation in social, leisure, and educational activities within the local area.
- Help learners access work experience and employment opportunities.
- Track and monitor learners' progress to ensure they receive personalised support.
