The Role

We are seeking an experienced Team Leader to join our multidisciplinary Home Assessment Team in North Lanarkshire. As part of the Scottish Government Discharge Without Delay agenda and Home First initiative, our team aims to reduce delayed hospital discharges by assessing needs and providing a period of rehabilitation in the home setting.

The Home First pathway encourages all health and care professionals to ask the questions 'why not home, why not now?' at every stage and has been evidenced nationally to result in significantly improved outcomes with reduced risk of deterioration in function through unnecessary prolonged hospital admission.

As a Team Leader, you will be responsible for operationally leading the Allied Health Professionals in the North West Sector Team, working closely with a North East Sector Team Leader to ensure North Lanarkshire wide cover with subsequent cross-cover as required.

Key Responsibilities

  • Lead and manage a team of Allied Health Professionals in the North West Sector
  • Develop and implement rehabilitation plans in the home setting
  • Collaborate with health and care professionals to reduce delayed hospital discharges
  • Provide leadership and guidance to team members
  • Ensure effective communication and coordination with stakeholders

Requirements

  • HCPC registration and either an Occupational Therapy or Physiotherapy background
  • Previous experience working in a community or patient/individual setting
  • Specialist knowledge and experience of relevant conditions, pathology, policies and procedures
  • Comprehensive knowledge of clinical guidelines and standards
  • Leadership, management, organisational and influencing skills
