Job Summary:
We are seeking a skilled and dedicated Case Officer to join our Short-Term Pathways Team in East Kent. As a Case Officer, you will work with adults over 18 with a wide range of needs, managing a small caseload and providing assessment, review, and arrangement of ongoing support to achieve improved outcomes and promote independence.
Key Responsibilities:
- Assess and review the needs of adults with complex needs, developing and implementing personalized care plans.
- Work collaboratively with health colleagues and other partnership agencies to promote independence and well-being.
- Provide ongoing support and guidance to individuals, families, and carers to ensure effective care and support.
- Travel across a wide geographical area to meet the needs of service users, working flexibly and at varying times of the day.
