Job Summary:The Case Officer will work with adults over 18 with a wide range of needs, managing a small caseload requiring assessment, review, and arrangement of ongoing support to achieve improved outcomes and promote independence.
Key Responsibilities:- Assess and review the needs of adults with complex needs, developing and implementing personalized care plans.
- Work collaboratively with health colleagues and other partnership agencies to promote a person's independence.
- Travel across a wide geographical area in a timely and flexible manner to meet the needs of service users.
- Contribute to the development and implementation of modernized office processes and performance management systems.
- Full induction and comprehensive professional learning and development.
- Regular and supportive supervision.
- Starting annual leave allowance of 27 to 29 days.
The ability to travel across a wide geographical area in a timely and flexible manner at various times of the day is essential. The new Adult Social Care and Health service operates with modernized office processes and performance management systems. It is increasingly accessible to the public outside of traditional office hours, as we move towards an 8am to 8pm, 7 days a week service to ensure service needs are met.
This post is considered by KCC to be a customer-facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements. The appropriate standards are set out in the Job Description/Person Specification.
