Case Officer

Job Summary:
We are seeking a highly motivated and experienced Case Officer to join our Short-Term Pathways Team. As a Case Officer, you will be responsible for managing a small caseload of adults with complex needs, providing assessment, review, and arrangement of ongoing support to achieve improved outcomes and promote independence.
Key Responsibilities:
- Assess and review the needs of adults with complex needs
- Develop and implement care plans to promote independence and well-being
- Work collaboratively with health colleagues and other partnership agencies to achieve improved outcomes
- Provide support and guidance to individuals and their families
- Manage a small caseload of adults with complex needs
Requirements:
- Experience of working with adults with complex needs
- Ability to assess and review the needs of adults with complex needs
- Excellent communication and interpersonal skills
- Ability to work collaboratively with health colleagues and other partnership agencies
- Ability to manage a small caseload of adults with complex needs
