Case Officer

About the Role
We are seeking a skilled and dedicated Case Officer to join our Short-Term Pathways Team in East Kent. As a Case Officer, you will work with adults over 18 with a wide range of needs, managing a small caseload and providing assessment, review, and arrangement of ongoing support to achieve improved outcomes and promote independence.
Key Responsibilities
- Assess and review the needs of adults with complex needs, developing and implementing personalized support plans.
- Work collaboratively with health colleagues and other partnership agencies to promote independence and well-being.
- Travel across a wide geographical area to meet with service users, providing timely and flexible support.
- Contribute to the development of modernized office processes and performance management systems, ensuring seamless service delivery.

Requirements
