About the Role

We are seeking a highly skilled and experienced Clinical Pharmacist to join our team at Hawthorn Medical Centre. As a key member of our primary care team, you will play a vital role in optimizing medication management for our patients.

Key Responsibilities
  • Conduct structured medication reviews (SMRs) for patients, ensuring that any necessary monitoring is completed or arranged.
  • Manage long-term conditions, including blood pressure control and other cardiovascular risk factors, and make changes to therapy as needed.
  • Provide education and support to patients on the use of their medications and optimize their health.
  • Handle day-to-day medication issues, including alternatives for out-of-stock items and clinical assessment of need for items not on repeat prescription.
  • Support the pharmacy technicians with clinical decision-making and identify patients suitable for electronic repeat dispensing (eRD).
  • Reconcile medicines on patients, record from discharge letters and outpatient clinic letters, and provide quality assurance of the process.
  • Assess patients new to the PCN to review their medicines and care, and refer into hospital services as appropriate.
  • Work with the PCN pharmacy team to collate data for medication optimization, conduct audits, and prepare reports.
  • Develop and deliver training for other staff on medicine-related themes and contribute to the development of PCN-wide policies and protocols.
Requirements
  • Mandatory registration with the General Pharmaceutical Council (GPhC)
  • Masters degree in pharmacy (MPharm) or equivalent
  • Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience
  • Excellent interpersonal, influencing, and negotiating skills
  • Excellent written and verbal communication skills
  • Ability to analyze and interpret prescribing data
  • Attention to detail and ability to work accurately
  • Ability to think analytically and anticipate obstacles
