Care Team Coordinator

Established in 2001, Radis Community Care has grown to become a leading provider of community-based social care and support for thousands of vulnerable adults and children in England and Wales. Our care ranges from brief daily visits to 24-hour live-in care and can either be short or long-term.
We care for a range of service users including the elderly and people with physical and/or learning disabilities. Tailored to meet our service user's individual needs, our care provides a unique service which is consistent throughout their life.
Job DescriptionThe successful candidate will work as part of a supportive team, providing person-centred care and support to individuals with complex needs. They will oversee the smooth running of services, complete regular audits and checks, and update and review support plans to ensure individuals achieve their outcomes.
Requirements- Previous experience working with individuals with Learning Disabilities, autism and mental health issues
- Previous experience as a Team Leader will be advantageous but not essential
- NVQ Level 2 in Health and Social Care and willingness to work towards Level 3
- NESB pension (with contributions from Radis)
