Job Title: Mental Health Support Worker

We are seeking a dedicated and compassionate Mental Health Support Worker to join our team at Gray Healthcare. As a Mental Health Support Worker, you will play a vital role in supporting individuals with complex needs, disabilities, and long-term conditions.

About the Role:

As a Mental Health Support Worker, you will be responsible for providing one-to-one support to an individual with a diagnosis of psychosis. You will work closely with the individual to develop a personalized support plan, promoting their independence, well-being, and quality of life.

Key Responsibilities:

  • Providing emotional support and encouragement to the individual
  • Assisting with daily living tasks, such as personal care and medication management
  • Supporting the individual to engage in activities they enjoy, such as outdoor walks and swimming
  • Implementing detailed care plans and risk assessments to ensure the individual's safety and well-being
  • Working collaboratively with the multi-disciplinary team to provide comprehensive support

Requirements:

  • Experience in social care, ideally within mental health
  • Ability to provide encouragement and support to individuals with complex health needs and learning disabilities
  • Experience in implementing detailed care plans and risk assessments
  • Current Right to Work in the UK and at least 6 months of employment in the UK
  • Enhanced DBS check and satisfactory references
