About the Role

We are seeking a skilled and dedicated Community Nurse to join our Felixstowe Community Healthcare Team. As a Community Nurse, you will play a vital role in providing high-quality nursing care to housebound residents of Felixstowe and surrounding areas.

Key Responsibilities
  • Triage, assess, plan, implement, and evaluate all aspects of nursing patient care, developing care plans that meet individual needs.
  • Ensure informed consent is obtained and documented prior to initiating interventions.
  • Develop and implement individualized, evidence-based care plans for patients, modifying them as required.
  • Assess patients' holistic needs, communicating complex and sensitive information to patients and carers.
  • Liaise with GPs, Social Services, inpatient teams, and other healthcare professionals to ensure identified needs are met and care coordinated.
  • Maintain skills with regards to latest technology and participate in multi-disciplinary/multi-agency meetings as appropriate.
  • Ensure own workload is planned and prioritized according to patient needs, and patient-held records are completed for each visit.
