We are seeking a skilled and compassionate Community Nurse to join our Felixstowe Community Healthcare Team. As a Community Nurse, you will play a vital role in providing high-quality nursing care to housebound residents in Felixstowe and surrounding areas.
Key Responsibilities- Triage, assess, plan, implement, and evaluate all aspects of nursing patient care, developing care plans that meet individual needs.
- Ensure informed consent is obtained and documented prior to initiating interventions.
- Conduct holistic assessments, devising individualized, evidence-based care plans that address physical, mental, and social health needs.
- Liaise with GPs, Social Services, inpatient teams, and other healthcare professionals to ensure coordinated care.
- Maintain skills in latest technology and participate in multi-disciplinary meetings as required.
- Ensure patient records are accurate and up-to-date, with daily inputting into SystmOne.
