Job Title: Advanced Community Nurse Practitioner

We are seeking an experienced and skilled Advanced Community Nurse Practitioner to join our team at Cambridgeshire Community Services NHS Trust. As a key member of our Integrated District Nursing Team, you will be responsible for providing high-quality, patient-centered care to individuals in the Luton community.

Job Summary

The successful candidate will have a strong background in community nursing, with experience in managing patients with acute and chronic long-term conditions. You will be a strategic thinker, able to lead and manage a team of healthcare professionals to deliver safe, effective, and responsive care to patients and their families.

Key Responsibilities
  • Provide leadership and management to a multidisciplinary community nursing team, ensuring the provision of high-quality, patient-centered care.
  • Develop and implement strategies to improve patient outcomes, reduce hospital admissions, and enhance the overall quality of care.
  • Work collaboratively with other healthcare professionals, agencies, and providers to ensure seamless care and effective communication.
  • Manage and coordinate the care of patients with complex needs, ensuring that their individual requirements are met and their dignity is respected.
  • Contribute to the development and implementation of policies, procedures, and guidelines that promote best practice in community nursing.
Requirements
  • Registered Nurse (RGN) or equivalent with current registration.
  • Specialist postgraduate education to degree level or equivalent in a relevant specialist area.
  • Evidence of ongoing continuing professional development (CPD) and commitment to staying up-to-date with best practice.
  • Strong leadership and management skills, with experience of managing a team of healthcare professionals.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and other healthcare professionals.
  • Ability to work autonomously and as part of a team, with a strong commitment to patient-centered care.
