Job Title: Advanced Clinical Practitioner - Virtual Wards

We are seeking two experienced Advanced Clinical Practitioners to join our team in developing and participating in the Virtual Ward service. This innovative model of care aims to deliver a collaborative, integrated approach to patient care, reducing hospital admissions and improving health outcomes.

About the Role

As a key Clinical Lead, you will be responsible for driving the successful management of the Virtual Ward service, working across the BLMK system and collaborating with multiple stakeholders. Your expertise will be essential in delivering high-quality care to patients, ensuring seamless transitions between community and acute care settings.

Main Responsibilities
  • Deliver expert clinical practice, including assessment, planning, and implementation of care plans
  • Provide professional leadership and consultancy to multidisciplinary teams, including ACPs, junior medical staff, and clinical nursing staff
  • Develop and implement education, training, and development programs for staff
  • Contribute to research, evaluation, and service development across the system
  • Work autonomously in delivering independent assessments for patients on the Virtual Ward
Requirements
  • Registered with NMC/HCPC
  • MSc Advanced Clinical Practice
  • Recent experience of working at an advanced practice level within Primary, Community, or Acute care
  • Extensive experience of advanced assessment, planning, diagnosis, and implementation
