About the Role
We are seeking a highly skilled and motivated Community Health Specialist to join our team at Wiltshire Health and Care LLP. As a Specialist Community Nurse, you will be responsible for delivering high-quality care to patients in their homes and community settings.
Main Responsibilities
- Lead a team of healthcare professionals in delivering community-based care
- Develop and implement care plans to meet the needs of patients
- Work closely with multidisciplinary teams to ensure seamless care delivery
- Provide education and training to patients and their families
Requirements
- Registered nurse with postgraduate community qualification
- Experience in community nursing and leadership
- Excellent communication and interpersonal skills
- Ability to work autonomously and as part of a team
