About the Role
We are seeking a highly skilled and motivated Community Health Specialist to join our team at Wiltshire Health and Care LLP. As a Specialist Community Nurse, you will be responsible for delivering high-quality care to patients in their homes and residential settings.
Main Responsibilities
- Lead a team of healthcare professionals in delivering community-based care
- Develop and implement care plans that meet the individual needs of patients
- Work closely with GPs, therapists, and other community services to ensure seamless care delivery
- Provide education and training to patients and their families on self-management of conditions
- Contribute to the development of community-specific training programs
Requirements
- Registered nurse with postgraduate community qualification
- Experience in front-line NHS community patient care
- Excellent clinical skills, well-developed communication skills, and ability to work under pressure
- Leadership experience and ability to work flexibly and efficiently
