About the Role

We are seeking a highly skilled and experienced Paediatric Community Nurse Practitioner to join our team at The Hillingdon Hospital NHS Foundation Trust. As a key member of our PATCH team, you will provide high-quality, compassionate care to children and their families in the community.

Key Responsibilities

  • Provide nurse-led care to children who would have otherwise been admitted to hospital, including telephone reviews and home visits.
  • Deliver health education and promotion to children and their families to reduce PED re-attendances and promote resourcefulness within the community.
  • Work closely with the lead nurse/practitioner, consultant, and multi-disciplinary team members to monitor and maintain policies, standards of nursing care, and staff development.
  • Be flexible in your approach to the clinical and community area and provide support for colleagues, junior staff, and students.

Requirements

  • Knowledge and experience of community nursing and paediatric care.
  • Skills and abilities/attributes, including experience teaching and supporting children and families.
  • Desirable: community experience, advanced paediatric nursing skills, and experience working with primary care.
