About the Role
We are seeking a skilled and compassionate Nurse Practitioner to join our innovative PATCH team at The Hillingdon Hospital NHS Foundation Trust. As a key member of our team, you will provide high-quality, individualized care to children and their families in the community, reducing the need for hospital admissions and promoting self-management of conditions.
Key Responsibilities
- Use your clinical expertise to assess and manage acutely unwell children, providing effective and compassionate care.
- Work closely with our multidisciplinary team to develop and refine your skills and knowledge, including opportunities to work in the Paediatric A&E Dept, Paediatric Assessment Unit, and children's inpatient ward.
- Support children and families in the community to self-manage conditions, reducing the need for hospital admissions and promoting hospital-at-home care.
- Assist in the reduction of acute admissions to inpatient hospital beds by providing hospital-at-home care and supporting children and families in the community.
- Work collaboratively with our lead nurse/practitioner, consultant, and multi-disciplinary team members to monitor and maintain policies, standards of nursing care, and staff development.
