The Cambridgeshire & Peterborough NHS Foundation Trust is seeking a dedicated Nurse for Adults with Learning Disabilities to join their Community Long Term Team in Peterborough. This exciting opportunity involves working closely with the Cambridgeshire Learning Disability Partnership to provide integrated health and social care services for adults with Learning Disabilities living in Cambridgeshire and Peterborough.

As a Community Health Practitioner, you will play a vital role in ensuring that assessments and provisions of person-centred specialist health and social care services promote individuals' rights to choice, independence, health, and wellbeing. Your experience in community-based roles supporting people with Learning Disabilities will enable you to manage a caseload, providing assessment and interventions related to the health needs of adults with a Learning Disability.

The successful candidate will have excellent organisation and time management skills, be able to reflect and critically appraise their own performance, and work effectively as part of a multidisciplinary team. You will also have knowledge of mental health legislation, IT skills, and an understanding of research and audit processes.

We estimate the salary for this position to be around £35,000 - £45,000 per annum, depending on experience.

Main Duties
  • Delivery of specialist nursing within Peterborough, working alongside colleagues within LDP of Cambridgeshire
  • Participation in the delivery of a countywide specialist Learning Disability nursing service under the guidance of a senior Nurse
