We are seeking an experienced Care Planning Specialist to join our dedicated team in Peterborough.
The successful candidate will have a solid understanding of care planning and coordination, with excellent problem-solving skills and the ability to manage a caseload of care packages effectively.
This is an office-based role, Monday to Friday, 9am – 5pm, with occasional travel across the East & South East region required. A valid driving licence and access to a vehicle are essential.
As a skilled Care Planning Specialist, you will be responsible for managing rotas for care teams, responding efficiently to day-to-day changes, and acting as the first point of contact for service users, their families, and other healthcare professionals.
You will apply your gained communication and relationship-building skills to build strong relationships with service users, their families, and other healthcare professionals.
A competitive salary between £26,000 - £28,000 per annum is offered, plus payment for on-call duty. There is also the opportunity to earn a bonus of up to £2,000pa.
