About the Role:

We are seeking a highly skilled and experienced social worker to join our Hospital Social Work team at Liquid Personnel. The successful candidate will work closely with healthcare professionals across acute and intermediate care settings, conducting assessments and contributing to complex discharge planning.

Responsibilities:

  • Conduct thorough assessments and complete Care Act and Mental Capacity Assessments
  • Lead on best interest decision-making and contribute to effective discharge planning
  • Collaborate with health colleagues to achieve positive outcomes for service users
  • Conduct safeguarding investigations using a strengths-based approach

Requirements:

  • Social Work England registration
  • Eligibility to work in the UK
  • Degree-level qualification in Social Work
  • Minimum 12 months post-qualified experience as a Social Worker
