An exciting opportunity has arisen to join a multidisciplinary team operating in the community across the London Borough of Brent. The team uses holistic assertive outreach techniques to support service users to access mental health services and to secure long-term accommodation.

This role is situated within a new team that targets rough sleepers who are isolated and are experiencing high levels of distress. The team will support by providing a reconnection service for those service users with no housing, health, or social links to re-establish links with family/support networks, working closely with third sector reconnection services, as well as foreign embassies to facilitate timely repatriation.

Main Responsibilities:
  • To work as a member of the multidisciplinary team providing specialist services to rough sleepers attending the service/unit.
  • To support the team with data entry and administrative work, ensuring clinical records are updated.
  • To ensure an understanding and implementation of clinical governance.
  • To ensure that all work is performed in accordance with Trust policies and procedures, including the Serious Incident policy.
  • To ensure concerns are raised and escalated appropriately and outreach worker is able to link in with main teams where needed to seek guidance/hand over.

To be familiar with all legislation and policies that relate to the team and apply correctly in practice. This will include all housing legislation and government homelessness initiatives in addition to national and local mental health policies and directives.

To undertake planned, regular contact sessions at specific agencies e.g. Day Centres, Hostels.

To participate in multi-disciplinary meetings.

To develop close links with Primary Care Services, Voluntary Agencies, Temporary Housing Services, and other stakeholders.

To liaise and communicate clinical information with partner agencies on shared care clients.

To contribute to the promotion of Team cohesiveness, multidisciplinary working and at all times work collaboratively with colleagues in all parts of the Trust to achieve healthy and effective communication.

To participate in a range of partnership arrangements both internal and external to the service/unit while remaining within the boundaries of good practice.

Administration / Data

To maintain accurate and up to date documentation of all service clients records, ensuring the confidentiality of such records of information are in line with the Data Protection Act and Information Sharing Protocol.

  • To adhere to all Rough Sleepers service data recording policies.
  • To be responsible for ensuring monthly contract and other data reporting activities are submitted within agreed timescales.
  • To ensure that all local service administration systems are followed.

To ensure that client information is entered in the service database system and to ensure activity input to clinical system is accurate and timely.

  • To undertake joint project work with other team members and to co-ordinate this if required, including scheduling meetings and minute-taking.
  • To produce regular reports and statistics for monitoring and evaluation purposes.

Personal/Professional Development and Training:

  • To be directly accountable for own practice and to operate in line with the relevant code of professional conduct in relation to policies on direct client care.
  • To undertake Operational Supervision in accordance with local line management arrangements and to undertake Professional/Clinical Supervision as stated in the CNWL Supervision Policy, in relation to own clinical work and professional development plan.
  • To complete the CNWL annual appraisal/review process and personal development plans/objectives identifying own training needs.
  • To participate in training initiatives as required both within the Sector and the wider professional community.
  • To have a clear understanding of the National Service Framework Models of Care, its application and implementation in the case management of own key working clients.
Person Specification
  • Educated to diploma level or NVQ level 4 in health and social care.
  • Degree.
  • Motivational Interviewing/Relapse management training.
  • Harm reduction training.
Experience
  • Evidence of personal and professional development in Rough Sleepers settings.
  • Ability to treat patients with respect and dignity.
  • Demonstrates the ability to manage work under pressure.
  • Previous experience within an NHS or local authority setting.
  • Data Processing and analysis.
  • Audit.
Skills & Knowledge
  • Demonstrated ability to work within and show understanding of equal opportunities.
