About the Role

We are seeking an experienced Advanced Nurse Practitioner to join our team at Naomi House and Jacksplace, a leading UK charity providing palliative and end-of-life care for children and young people with life-limiting conditions.

Key Responsibilities
  • Assess, diagnose, and manage the health needs of children and young people with complex conditions, working autonomously and in conjunction with other professionals.
  • Develop and implement individualized care plans, incorporating risk assessments and holistic approaches to meet the needs of children and young people.
  • Provide clinical leadership and support to the multidisciplinary team, ensuring high standards of care and adherence to policies and guidelines.
  • Collaborate with other agencies and professionals to ensure seamless services and effective care coordination.
  • Contribute to the development and implementation of policies and procedures, ensuring compliance with regulatory requirements and best practice.
Requirements
  • Registered Nurse with valid NMC registration and eligibility to work in the UK.
  • Experience working with children and young people with complex needs, including palliative care.
  • Strong clinical skills, including medicine management, IV therapy, and knowledge of relevant guidance and policies.
  • Ability to work autonomously and as part of a team, with excellent communication and interpersonal skills.
