We are seeking a qualified Health Visitor to join our team at Southern Health NHS Foundation Trust. As a Health Visitor, you will play a vital role in delivering high-quality care to children and families in our community.
Key Responsibilities- Lead the delivery of the Healthy Child Programme, focusing on the 6 High Impact Areas, using evidence-based practice.
- Develop and implement preventative community-based approaches and personalized care to support the local population.
- Work collaboratively with multidisciplinary teams to assess and address the needs of children and families.
- Provide expert advice and guidance to colleagues and partners on health visiting best practices.
- Registered Nurse/Midwife with a BSc Hons Public Health Specialist Practitioner (Health Visiting) or equivalent qualification.
- NMC Registered Part 3.
- Full UK Driving Licence and access to a car.
