Person-Centered Social Care Professional

About the Role:

In this rewarding role, you will provide person-centered care and support to service users, ensuring their physical, emotional, and psychological needs are met. You will work collaboratively with colleagues to develop and implement personalized care plans that promote independence, dignity, and well-being.

Key Responsibilities:

  • Provide person-centered care and support to service users with neurological conditions
  • Develop and maintain effective relationships with service users, families, and healthcare professionals
  • Collaborate with multidisciplinary teams to design and deliver individualized care plans
  • Maintain accurate and detailed records of service user interactions and progress

Requirements:

To be successful in this role, you will need:

  • A degree in a relevant field (e.g., social work, nursing, psychology)
  • Proven experience working with individuals with neurological conditions
  • Excellent communication and interpersonal skills
  • Able to work independently and as part of a team
