About the Role

We are seeking a highly motivated and empathetic individual to join our team as a Rehabilitation Support Worker. As a key member of our team, you will play a vital role in supporting individuals with brain injuries to regain their independence and achieve their goals.

Key Responsibilities
  • Provide person-centred support to individuals with brain injuries, promoting their independence and well-being.
  • Work collaboratively with the rehabilitation team to develop and implement individualised care plans.
  • Assist individuals with daily living tasks, such as personal care and household chores.
  • Encourage and support individuals to participate in activities that promote their physical and emotional well-being.
  • Maintain accurate records of individual progress and contribute to the development of care plans.
