Clinical Lead

We are seeking an experienced Clinical Lead to join our team in Bracknell, where you will lead and support our community nurses. As a Clinical Lead, you will be responsible for providing enhanced clinical support and expertise to patients, implementing evidence-based practice to ensure best patient care.
About the RoleThe successful candidate will have a Specialist Community Nursing Qualification and hold an Independent Prescribing Qualification (V300) or be willing to undertake this within 6-12 months in post. You will work alongside the Operational Team Lead to ensure staff are competent in their role and new staff are inducted and supported to reach competency required for their role.
Main Duties- Ensure care in their locality is based on sound assessment and care planning processes.
- Develop and implement effective care pathways to support patients with complex needs.
- Provide education and training to staff to ensure they have the necessary skills and knowledge to deliver high-quality care.
- Monitor and evaluate the effectiveness of care services and make recommendations for improvement.
- Lead and manage a team of community nurses to ensure they are working effectively and efficiently to deliver high-quality care.
- Develop and maintain relationships with other healthcare professionals and agencies to ensure seamless care transitions.
- Ensure that all staff are aware of and adhere to the Trust's policies and procedures.
