Recovery Mentor for the Criminal Justice System

We are seeking a highly skilled and compassionate Recovery Mentor to join our team at The Forward Trust. As a Recovery Mentor, you will play a vital role in supporting individuals with substance misuse issues within the criminal justice system.
Job Summary:
Working closely with our team, you will provide individualized support and guidance to clients who have been referred to our services through the Criminal Justice system. Your primary goal will be to empower these individuals to make positive changes in their lives and break the cycle of addiction.
About the Role:
- Deliver comprehensive assessments, risk management plans, and SMART care plans tailored to each client's unique needs.
- Collaborate with various stakeholders, including CJ agencies, healthcare professionals, and community services, to ensure seamless continuity of care.
- Implement structured interventions and group-work sessions using evidence-based practices, such as motivational interviewing techniques.
- Maintain accurate records and reports, ensuring effective communication with all involved parties.
- Contribute to the achievement of targets set by Forward and local commissioners, focusing on measuring outcomes and co-operative working relationships with partner agencies.
Requirements:
- Experience working within substance misuse services and with CJ clients.
- Demonstrated ability to carry out comprehensive assessments, risk management plans, and SMART care plans.
- Strong understanding of continuity of care, holistic care needs, and community services.
- Familiarity with CJ agencies, including police, probation, CRC, and prisons.
- Excellent IT skills and ability to maintain accurate records.
- Knowledge of Safeguarding, Equality and Diversity, and the 12-step process of recovery from addiction.
